In both cases the points of failure appear to be well outside of our network. The file server is operating in a degraded state so is running with less performance than normal.&lt;br /&gt;&lt;br /&gt;We expect the process to be completed by 6pm.&lt;br /&gt;&lt;br /&gt;Until that time the mail servers will run with a higher than normal load and may be less responsive that usual.&lt;br /&gt;&lt;br /&gt;UPDATE 1pm Monday&lt;br /&gt;&lt;br /&gt;Work to bring the file server up to speed with a full disk set should now be completed by 4pm.  &lt;br /&gt;&lt;br /&gt;We have identified during the work that one of the POP cluster servers is suffering from a faulty network connection.  Further investigation will continue this afternoon.&lt;br /&gt;&lt;br /&gt;UPDATE 3 pm Monday&lt;br /&gt;&lt;br /&gt;The process to re silver the missing drive is due to complete at 4.30pm.&lt;br /&gt;&lt;br /&gt;During the monitoring of the server loads it has become increasingly apparent that there are a number of customers with POP accounts where mail is being stored for considerable periods of time.  POP is not best suited to storing mail for longer periods and it is apparent that the loads on the POP servers are being caused by inappropriate use of the POP accounts.&lt;br /&gt;&lt;br /&gt;We will be performing a review of POP usage over the next week and advising customers where their usage needs to be modified.&lt;br /&gt;&lt;br /&gt;UPDATE 8pm Monday&lt;br /&gt;&lt;br /&gt;File system completed the rebuilt at 4.30pm. We installed a UPS at 7,30pm last night on a switch that had been suffering from power cycling issues.&lt;br /&gt;&lt;br /&gt;This morning the transfer from generator to mains at a local POP appears to have caused one of our Capital Court switches to lock it's network facing uplink port. type='text'>Mysql1 Server</title><content type='html'>Thursday 7th Jan 4am.&lt;br /&gt;&lt;br /&gt;The sql server mysql1.nsnoc.com has crashed and requires a restart.  It has failed to respond to two automated reboot attempts and will require manual intervention to restart it.&lt;br /&gt;&lt;br /&gt;An engineer will be on site shortly to attempt to resolve the issue.&lt;br /&gt;&lt;br /&gt;Next update due 7am.&lt;br /&gt;&lt;br /&gt;UPDATE&lt;br /&gt;The server has responded to an engineer being present at the reboot. All services are working normally.&lt;br /&gt;&lt;br /&gt;It is not clear at this time why the server is not rebooting on the remote switch when required so we will take steps today to migrate the database to new hardware.&lt;br /&gt;&lt;br /&gt;UPDATE 2PM&lt;br /&gt;The cause of last nights issue has been traced to a faulty network card. The UPS maintained service to the remaining servers for 15 minutes but it took longer than anticipated to identify the issue and gain access to the building.  &lt;br /&gt;&lt;br /&gt;An engineer was on site at 3pm to identify the issue and start the process of finding the server that caused the initial trip.  &lt;br /&gt;&lt;br /&gt;Power was restored to most servers by 3.30pm.  We worked through all of the services to ensure that everything was working and by 5pm concluded that all remaining bugs had been resolved.&lt;br /&gt;&lt;br /&gt;During the process we identified that one of the core switches had been incorrectly powered from the rack B UPS so the failure impacted on some services from other racks. This issue will be resolved on 4th January when the office reopens.&lt;br /&gt;&lt;br /&gt;At this moment in time, the logs.nsnoc.com server is offline until a new power supply can be installed on 4th January. We will process the logs from today onwards once the server is working again.&lt;br /&gt;&lt;br /&gt;All other services have been tested and confirmed as working. Please contact us to renew the service and re enable your site.&lt;br /&gt;&lt;br /&gt;- A small number of sites have databases with some locked/crashed database tables. There is nothing to suggest these are active tables or that the issue has only recently occurred but if you do have a table that requires a repair please let us know.&lt;br /&gt;&lt;br /&gt;- The ftp server on the new host is being very 'Monday' and not authenticating requests at the moment. The pass expired 2 days ago thereby preventing us from completing the migration according to our original timetable.&lt;br /&gt;&lt;br /&gt;We have no alternative dates to complete the work so it will commence at 7.30 am this morning with a target completion of 9.30am.&lt;br /&gt;&lt;br /&gt;UPDATE 11AM&lt;br /&gt;We're seeing high traffic levels at the file server caused by a single site.  If services do not settle to an acceptable level within the next 5 to 10 minutes we will disable the site and look for stability to return to the servers.&lt;br /&gt;&lt;br /&gt;UPDATE 1.30PM&lt;br /&gt;We have been able to confirm the issues seen after the file server was returned to service this morning we caused by a single site. Until the repair is completed there is no access to POP, IMAP, or Webmail services.&lt;br /&gt;&lt;br /&gt;&lt;span style="font-weight:bold;"&gt;UPDATE 10:15PM&lt;/span&gt;&lt;br /&gt;The file system check and repair has completed however a server reboot is required to clear a number of Dead/Zombie processes on the server.&lt;br /&gt;&lt;br /&gt;We're going to assess the issues and decide on a course of action shortly.&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;&lt;span style="font-weight:bold;"&gt;UPDATE 10:50PM&lt;/span&gt;&lt;br /&gt;An engineer will be dispatched to IFL2 at 3am to recover the file server to our new racks.  We hope to have the service restored shortly after 8am.&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;&lt;span style="font-weight:bold;"&gt;UPDATE 8.15AM&lt;/span&gt;&lt;br /&gt;The overnight work to recover the file server to our new racks was completed on time and without incident.&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;&lt;span style="font-weight:bold;"&gt;UPDATE 11:45AM&lt;/span&gt;&lt;br /&gt;Heavy storage loads have resulted in the need to pull the file system and force a tree rebuild - this is going to be a lengthy process, so we are working to restore a backup in the interim with an ETA of a fifth of the time before inboxes are present. AGAIN I would like to reiterate this is not preventing inbound mail from arriving, merely the ability to collect it, an important difference.&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;&lt;span style="font-weight:bold;"&gt;UPDATE 13:38&lt;/span&gt;&lt;br /&gt;Working on a solution to deliver / present fresh email to the end users. While historical data will be missing, all new emails will be arriving and visible - allowing us to migrate these to the existing file system once rebuilt. Light at end of collective tunnels.&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;&lt;span style="font-weight:bold;"&gt;UPDATE 13:45&lt;/span&gt;&lt;br /&gt;Temporary solution in place. POP users will now be seeing all their new email arriving. Those who are paying for the IMAP solution will notice that all their mail has gone. DO NOT adjust your set. It would appear that a complication with files not being deleted properly after being cleared from the spool queue had arisen, and as a result on recovering these files to a state in which they could be deleted - the mail server diligently sent them on their way. We will monitor the network over the next 24 hours to ensure that we can maintain a degree of stability.&lt;br /&gt;&lt;br /&gt;UPDATE  19:00&lt;br /&gt;&lt;br /&gt;It has become apparent this afternoon that a second denial of service attack was hitting our DNS servers ns1 and ns2.  Customers will be aware of the importance of the name servers.  Not only do they allow remote visitors to find the IP address of the server, they're also used by the web servers and Sql servers for performing reverse lookups. &lt;br /&gt;&lt;br /&gt;The result of the attack has been to slow down all services including DSL/transit due to the high volume of UDP traffic hitting our routers.&lt;br /&gt;&lt;br /&gt;We believe that at this stage we have been able to take action to reduce the effects of the Dos attack. We will be monitoring the network this evening and taking appropriate action as needed.&lt;br /&gt;&lt;br /&gt;UPDATE 19:50&lt;br /&gt;Network monitoring is showing that services have returned to normal.  We will continue to monitor the network and services over the next 24 hours.&lt;br /&gt;&lt;br /&gt;UPDATE 21:00&lt;br /&gt;One issue with some static transit routes has been resolved. The result being that sites on clusters 150, 156 and 212 have suffered 3 outages in the last hour lasting up to 10 minutes.&lt;br /&gt;&lt;br /&gt;Work is nearly completed to complete the router installation. Further updates will follow.&lt;br /&gt;&lt;br /&gt;UPDATE 18:00&lt;br /&gt;&lt;br /&gt;The work to replace router 2 was completed earlier this afternoon but is not yet in use.  We will shedule out of hours work to enable the upstream connections on this router to reduce disruption during the settlment of our routing tables.&lt;br /&gt;&lt;br /&gt;We have identified two issues during the work today.&lt;br /&gt;&lt;br /&gt;An APC power switch which supports the load balancer for 150, 156 and 212 appears to have a loose connection which causes the load balancer, servers 29, 37 and 64 to reboot if the cables are moved.  We will shedule another maintenance window to transfer the power cables to a new APC unit. As yet a date has not been set.&lt;br /&gt;&lt;br /&gt;The second issue relates to a fault with a link to IFL1 which carries our ADSL customer traffic.  The work on the new router today has highlighted a potential fault which we have raised with the link provider today. Downloads appear to be looping.&lt;br /&gt;&lt;br /&gt;To rule out any corruptions to the upstream cache at the mail server, the local index has been removed. While this restructures there will be some delay.&lt;br /&gt;&lt;br /&gt;If your mail client is repeatedly downloading email please take it off line. Further information will be made available as we have it.&lt;br /&gt;&lt;br /&gt;UPDATE&lt;br /&gt;&lt;br /&gt;It would appear that we have a rouge AS advertisement being made by an ISP not providing service to us.  we are attempting to work through the various channels to have their network management centre correct the issue.&lt;br /&gt;&lt;br /&gt;UPDATE&lt;br /&gt;&lt;br /&gt;A ticket has been raised with the noc of the ISP concerned we are awaiting the results of their investigations.&lt;br /&gt;&lt;br /&gt;UPDATE&lt;br /&gt;&lt;br /&gt;The issue has been identified relating to a single upstream provider so engineering are now working to ensure that the fixes are put in place with our supplier network to restore connectivity via that route.&lt;br /&gt;&lt;br /&gt;Our network stats are showing working routes via two out of three upstream providers and all of our Manap peering. That should be completed shortly.&lt;br /&gt;&lt;br /&gt;UPDATE&lt;br /&gt;&lt;br /&gt;Mail storage services are all restored and working again. While this may make sense on the scale of one person, upscale this a couple of hundred times, and there is a fair amount of mail happening that doesnt need to. &lt;br /&gt;&lt;br /&gt;We are aware of the issue, and we are processing the queue as well as we are able - once the queue has returned below 8K mails, we are confident it will start to recover of its own accord. Bear with us, and we will update you once we have the green light.&lt;br /&gt;&lt;br /&gt;&lt;span style="font-weight:bold;"&gt;[update 0955]&lt;/span&gt; Blaming the tail end of an issue yesterday for this mornings issues quickly became a non starter. Cluster 2 and 3 are operating within tolerances if a little slow.&lt;br /&gt;&lt;br /&gt;We are currently looking into storage issues, as yesterday an air conditioning failure in the data centre IFL2 resulted in a sizable heat spike for a prolonged period.&lt;br /&gt;&lt;br /&gt;UPDATE 17:00&lt;br /&gt;&lt;br /&gt;The cause of the issue has been traced to the file server and the underlying cause identified as an overheating CPU. While the service is working better than it was earlier, the process list on the web servers is continuing to climb to the point that new connections are not permitted.&lt;br /&gt;&lt;br /&gt;At this point we have eliminated the possibility that the fault is within the LVS load balancer (that was replaced this morning) and the SQL server (replaced 1pm with a new server).  We have also eliminated the network as the possible source of the issue as virtual servers and the mail service are working without issue.&lt;br /&gt;&lt;br /&gt;The only other element of the service which is now in question is the nfs file server.  While there are no obvious errors being produced we feel that it is the only possible cause of the issues left.  A new file server is in the rack and we have just begun the process of transferring of data from the old server to the new.  We expect that to be substantially completed within the next 4 hours.&lt;br /&gt;&lt;br /&gt;UPDATE 8PM&lt;br /&gt;&lt;br /&gt;The transfer of files to a new file server is underway and proceeding without issues.  The web servers have been pointed to the new file server.  Files are being restored from a to z so sites starting a and b have already been migrated.  Judging by the first hour of transfer, we expect the process to complete in the early hours of the morning.&lt;br /&gt;&lt;br /&gt;We would like to thank customers for their patience during this time.&lt;br /&gt;&lt;br /&gt;UPDATE 1AM&lt;br /&gt;&lt;br /&gt;We are approaching half way through the transfer of sites from the old file server to the new.  We expect the remainder of the process to be completed by 6 to 8am.&lt;br /&gt;&lt;br /&gt;Webmail services have been restored and are working without issue.&lt;br /&gt;&lt;br /&gt;FTP access to the new file server will be suspended until mid morning Wednesday.&lt;br /&gt;&lt;br /&gt;UPDATE 7AM&lt;br /&gt;&lt;br /&gt;The file transfer is still running with about 75% of sites completed allbeit very slowly. Clients with sites still not available can email our support email address with any sites not showing so that we can push them by hand.  Priority will be given to business sites.&lt;br /&gt;&lt;br /&gt;UPDATE 11 AM&lt;br /&gt;&lt;br /&gt;All of the remaining sites should have been resored within the next 2 hours. Once that is complete, ftp access will be made available to the new file server.  Customers will not need to change any settings in their ftp clients.&lt;br /&gt;&lt;br /&gt;UPDATE 2PM&lt;br /&gt;&lt;br /&gt;All transfers are completed and there appears to be stability at last.  A few bugs with sites have cropped up during the process but they have been ironed out. The file server appears to be operating correctly but as it is such a major component in the delivery of the web pages, and given that we have no other potential sources for the problem we have decided to replace the file server as a precaution.  We expect that work to be completed by lunchtime today.&lt;br /&gt;&lt;br /&gt;UPDATE 9.40am&lt;br /&gt;&lt;br /&gt;Further testing has identified that the bottleneck appears to be the SQL server not the File server as previously thought.  Work to replace the file server has been suspended for the time being.  We have a standby version 5 SQL server and are currently investigating the issues involved in making an upgrade from the current mysql4 service to mysql5 on the new server.  If the issues appear to carry too high a risk, then we will prepare a Mysql 4 server.  This work is expected to last for the next 3 hours.&lt;br /&gt;&lt;br /&gt;UPDATE 1pm&lt;br /&gt;&lt;br /&gt;The database tables and indexes on the existing sql server have all been repaired but this has not given us any significant performance increase.  A new Mysql 4 server is being prepared to take the databases.  Next update 2pm.&lt;br /&gt;&lt;br /&gt;UPDATE 2PM&lt;br /&gt;&lt;br /&gt;The new database server should be running within the next 10 to 15 minutes.&lt;br /&gt;&lt;br /&gt;UPDATE 3PM&lt;br /&gt;&lt;br /&gt;The main database has now migrated to a new multi core host server. We are aware of this. No inbound mail is being lost, however you will currently be unable to get to it until we have repaired the drive; this includes the webmail facility. For the users this involves the data has been set read only to prevent damage to the inboxes. We will update you as soon as we are able.&lt;br /&gt;&lt;span style="font-weight:bold;"&gt;[Update 0934]&lt;/span&gt;&lt;br /&gt;We have recovered the data, and are brining live mail collection within the next five minutes. Inbound mail will be live hopefully within the next 20 minutes. Replacement of device did not resolve the issue. Realisation of the intended target and steps taken to minimise the impact reduced the overhead, however removal of the target machine resulted in an immediate return of service.&lt;br /&gt;&lt;br /&gt;Fall back service provision will be removed during the course of the morning to return to full primary service. We have had an influx from a spamming attempt, and are taking steps to remove these from the live queue and locate the source and take steps to prevent this flow before restarting the service.&lt;br /&gt;&lt;br /&gt;&lt;span style="font-weight:bold;"&gt;[Update 1003]&lt;/span&gt;&lt;br /&gt;&lt;span style="font-style:italic;"&gt;The subsequent work has revealed an underlying file structure issue with a host machine serving data for a number of back office services. Taking it out of service users will have experienced sporadic connectivity to the mail server for a five minute period during the diagnosis and change over. While this is not the case, it is not preventing them from currently rejecting mail forwarded to accounts on their servers from our clusters.&lt;br /&gt;&lt;br /&gt;Feedback has been along the lines of:&lt;br /&gt;&lt;br /&gt;&lt;span style="font-weight:bold;"&gt;[snip]&lt;/span&gt;&lt;span style="font-style:italic;"&gt;I'm sorry to hear you have been adversely affected by changed we have made to our email delivery system.  The changes were designed to reduce the amount of spam our network receives.  This is a generally accepted practice in the industry.&lt;br /&gt;&lt;br /&gt;No hosting service should be using dynamic IPs to forward mail, you might want to discuss this with them. The new routing table should then be updated to other routers that peer with it thereby preventing traffic being passed off to routers which cannot handle it.&lt;br /&gt;&lt;br /&gt;We have closed down the peer with the London provider concerned and have raised the issue with them. This will be effecting both mail and web will be effected for some clients, others will see no impact what so ever. This is now the 5th block in as many months.&lt;br /&gt;&lt;br /&gt;AOL have refused to whitelist the IP addresses of our mail servers.&lt;br /&gt;&lt;br /&gt;AOL customers now have two options to ensure that they are able to have email services on their domain names.&lt;br /&gt;&lt;br /&gt;1) Migrate your ADSL service away from AOL to ourselves. After a fibre break on the primary path, the secondary path has suffered a flooded repeater module in the Shefield area.&lt;br /&gt;&lt;br /&gt;We continue to offer service without interruption or degradation through our other peers and providers. This is apparently a product of higher external temperatures and air conditioning filters becoming clogged with pollen / Dandelion seeds.&lt;br /&gt;&lt;br /&gt;We monitor internal case and hard drive temperatures along with services around the clock, and are able to see trends when we are experiencing issues with our remote servers.&lt;br /&gt;&lt;br /&gt;We are working to ensure that server losses are worked around, and that any down times are reduced to an absolute minimum. We are currently taking steps to replace the damaged cache. Rather than letting it repopulate itself we are intending to seed the cache to reduce the impact to you.&lt;br /&gt;&lt;br /&gt;This will currently be showing to an unfortunate few as a replicated mail, or mail you cannot delete or read. While we are trying to keep the service up as much as possible, we are aware there are going to be perceived outages.&lt;br /&gt;&lt;br /&gt;Every effort is being made to maintain service, however we are increasingly aware that this maybe due to malicious intent, and are looking into this currently. If you have a support query regarding DNS or lack of resolution, hold fire, and we will update this as often as we are able.&lt;br /&gt;&lt;br /&gt;In overview a lack of name resolution for a number of the 64,000 domains we host is currently occurring.&lt;br /&gt;&lt;br /&gt;&lt;b&gt;[Update 14th May 12:30]&lt;/b&gt;&lt;br /&gt;&lt;br /&gt;The issue has been dealt with before the close of Saturday. The secondary name server has been shut down, and a new one built configured and deployed.&lt;br /&gt;&lt;br /&gt;This event bought to our attention that an older DNS server that many users have used on their DNS records was no longer valid. The mechanism that proxies the RADIUS authentication off of the BT network is not functioning. While we are seeing the requests from authentication and granting them where correct, this data is not getting back to you.&lt;br /&gt;&lt;br /&gt;This issue is currently seeing as being within the Thus/Demon/YourComms network and a solution from them is being persued. The issue was first discovered at 10am today. Once the 5 minute delay  has expired email from the host/domain is accepted immediately and will continue  through the usual scanning processes.&lt;br /&gt;&lt;br /&gt;A normal mail server sending mail  will have no problems in retrying the mail delivery after 5 minutes has expired.  Where the inbound mail is being sent from a compromised server or PC, the spam  software does not normally run a queue for delayed mail and so the mail is never  resent.  The statistics of the last 48 hours suggest as many as 50% of the  servers attempting to deliver mail which pass the RBL tests are not genuine mail  servers as they never attempt to deliver the mail a second  time.&lt;br /&gt;&lt;br /&gt;Customers should be aware that mail will only be delayed by 5  minutes where that sender has never sent mail to our network previously.  Our  servers have already whitelisted many popular servers and mail addresses  including hotmail, msn, yahoo and ebay.&lt;br /&gt;&lt;br /&gt;For customers still seeing  occasional spam mails it is worth ensuring that you do not have a catchall on  your mail address. Where a catchall is unavoidable, identifying specific aliases  that are targeted by spammers is a useful option. These aliases can then be set  via our support site with a target of :fail: (include the colons) which will  cause all future mail to those aliases to be rejected to the sending  server.&lt;br /&gt;&lt;br /&gt;Finally it is worth noting that the vast majority of the spam  that we see can be traced back to mailto: links on websites that have been  harvested by spammers.
